Change of Circumstance Form

Parents will need to complete this form if anyone else is due to collect their child. This system is part of our safeguarding procedures and ensures all children are only ever dismissed to approved adults. It is perfectly fine for parents to complete this form for repeat collection (eg. Grandma (her name) collects (named child) every Tuesday and Thursday / Frank’s mum (name) will be collecting (your child’s name) every Monday.)

Exceptional Absence Request Form

Parents will need to complete this form in advance of any leave of absence (it does not need to be completed if your child is ill. In this instance, please call the school office to report your child’s non-attendance on the morning of the absence). Parents are asked to read our attendance leaflet and attendance policy ahead of making a request.
